Changes:

Tournament entry fees are now refunded in the crash recovery feature. This is now consistent with stopping a tournament manually, where the entry fee has always been refunded to the players still remaining in tournament. Players that had already busted out of the tournament before the stoppage are not refunded.

Fixed a minimum raise bug. Previously if a player raised all-in but did not create a full raise, the next player would get an incorrect (too low) minimum value on his slider bar.

Improved handling of socket thread errors.
